Personally, I have only been to a bed and breakfast in the country. And to be honest, I loved how quaint it was and its many charming qualities ... but on the other hand, we were so far from LIFE and things to do, that I was quite bored. I think that the next time I do select a B&B it would be located in a city. Hotels can be too generic. If you go to a B&B in the city, you have the best of both worlds: feeling comfortable in your accommodations with great food -- not to mention the personal mark it leaves you with -- plus being close to the things you visited for.
I, too, have only stayed in country B&B's. However, I have stayed at smaller, boutique hotels (one example is the Art Hotel in Buenos Aires) that offer more personalized service and feel a little less hectic than the big chains that seem to dominate major cities. I would definitely be open to staying at a big-city B&B, especially for cities I've already been to. I've done the posh Manhattan high-rise thing ... it would be nice to try a weekend in the city where you come "home" someplace.
Two come to mind straight away: The Chelsea Inn and The Larchmont (though the latter may fall into the hotel category, but I swear, it "feels" like a B&B).
The Chelsea Inn was so country-cozy that I lost sight of being in the big city! I remember my room being on the smallish side, but very clean and tastefully decorated. It was actually furnished with mismatched antiques -- and I'm a sucker for that. I shared a bathroom with another room of guests, but still, it was spotless. And I just loved the funky murals painted on the bathroom walls. Folks at the Inn were friendly and I felt more like I was staying at my rich friend's house than at a B&B. Overall, very cute.
The Larchmont is my most favorite place to stay in NY and I love love love it! It is so well-priced for *all* that you get (slippers, robe and the sensation of being a HIP city person) and is conveniently located by good eats, drinks, shops, etc. Each floor has a few shared bathrooms plus a communal kitchen, and all have been spotless on my past stays. Service has always been good, no complaints. And I guess its catching on: I recommended it to friends and they couldn't get a room even though it was about 7 or 8 weeks in advance.

The 1871 House in NYC is great -- beautiful townhouse in a great neighborhood just a (very) short walk to Central Park. Perfect!
